Max, my male dog yorkshire terrier, has diarrhea, bloody stool, and vomit

My dog is taking iverhart for heartworm every month but I noticed today my dog stool is bloody and loose stool and I know my dog eats grass because he vomit grass but just wanted to know about the booldy stool
